Implementing the chronic care model in an academic setting: a resident's perspective Coca A, Francis MD
Most current health systems are poorly designed to provide optimal care for patients with chronic illness. The chronic care model has evolved from an interest to correct these health system deficiencies. The aim of the Academic Chronic Care Collaborative (ACCC) is to foster adoption of the chronic care model in academic settings and to promote clinical education about optimal care for chronic illness. Southern Illinois University (SIU) is 1 of 22 academic medical centers participating in the collaborative. This article reports the experience of 3 internal medicine residents who had a primary role in the pilot phase of the ACCC project at SIU.
