Diabetes disease management at Lovelace Health Systems: theory and implementation Friedman N, Shainline L, Foris M, Cypress M, Gleeson J
Lovelace Health Systems (LHS) and Lovelace Healthcare Innovations, Inc. (LHI), its disease management subsidiary, have developed 19 disease management programs that describe optimal processes of care for acute or chronic conditions across the continuum of care, from diagnostic screening to long-term treatment. Using details from the LHS/LHI diabetes program, the authors describe the steps in both the development and implementation of disease management programs, including selecting conditions and setting program objectives, defining outcomes measures and designing outcomes tracking tools, and developing guidelines and other aids to effective management. The authors also discuss practical means of communicating results to providers as well as barriers to successful program implementation.
