This patient has dementia with Lewy bodies, which is a Parkinson plus syndrome. The central feature of dementia with Lewy bodies is progressive cognitive decline in addition to 3 defining features: pronounced “fluctuations” in alertness and attention, recurrent visual hallucinations, and parkinsonian motor symptoms.
