This patient has Fitz-Hugh-Curtis syndrome, an acute perihepatitis caused by direct extension of N. gonorrhea or C. trachomatis from the fallopian tube to the liver capsule and peritoneum. Herpes, chancroid, and syphilis would not spread to the peritoneum.
