Client injuries incurred in 24-hour behavioral health care settings: a systematic evalutatoin and method of examination

Abstract Objective: To examine client injuries in residential behavioral health care settings. Methods: We developed an injury intensity model and a method of categorization for tracking and recording client injuries incurred across all ages, diagnostic categories, and settings of care. The relationship of injury type to demographic risk factors such as age, gender, diagnosis, and injury severity was evaluated. Contextual risk factors such as setting of care, time of day, time of year, staff involvement, and intervention type was also considered. Results: Statistically significant associations were seen between demographic variables and injury type and severity, and the severity of injuries with the source of injury and restraint and device use. We also found differences in the area of the body affected by the injury when results were stratified by client gender. The highest percentages of severe injury were accidental and self-injurious behavior–related, while the lowest percentages were client-on-client and restraint-based injury. Conclusion: There were differences in the severity of injury based on the source and restraint use. This research also presents a valid and reliable instrument for other organizations to use when documenting client injuries to standardize internal quality assessment.
