The follow-up service: easing the transition from the inpatient to the outpatient setting Pantilat SZ, Kerr K, Goldman L, Wachter RW
This article describes a service that allows inpatient physicians to schedule an outpatient visit for patients who require re-evaluation within 2 days of discharge but have no primary care physician or are otherwise at risk for a lapse in care. Physicians and patients reported being satisfied with the service and feeling more comfortable with discharge knowing a visit had been scheduled.
