Reversing warfarin anticoagulation: utilization of fresh frozen plasma and vitamin K in a community hospital Wu BJ, Joshi K, Benis LJ
The authors report on physician adherence to national guidelines for management of excessive anticoagulation. 95% of patients who received fresh frozen plasma had an indication for reversal, but most received twice the recommended dose. Only 2 of 3 patients with an indication received vitamin K, and none received vitamin K at the dose and via the route of administration specified in the guidelines.
